The terminology around fertility treatment can be confusing. IVF (in-vitro fertilization) is the umbrella term for a process that involves fertilizing eggs with sperm outside the body. ICSI (intracytoplasmic sperm injection) is a specific technique used during the IVF process, where a single sperm is injected directly into an egg.

The standard IVF approach

In conventional IVF, an embryologist places thousands of sperm in a culture dish with each egg and allows fertilization to occur naturally. Sperm compete to penetrate the egg, and the strongest swimmers typically succeed.

How ICSI differs

In ICSI, the embryologist selects a single sperm, immobilizes it, and injects it directly into the egg cytoplasm using a fine glass needle under a microscope. This bypasses the natural sperm-egg interaction entirely.

When is ICSI needed?

ICSI is now used in roughly 60-70% of all IVF cycles globally, even when male factor infertility is not present. The decision typically rests on:

  • Sperm count and motility — ICSI is recommended when counts are below 5-10 million/mL or motility is severely reduced
  • Previous fertilization failure — if a previous IVF cycle had low or zero fertilization, ICSI is usually recommended
  • Use of frozen eggs — frozen eggs have a hardened zona pellucida that can make conventional fertilization difficult
  • Severe DNA fragmentation — high DNA fragmentation in semen may benefit from ICSI selection

Are success rates different?

For patients with normal sperm parameters and no history of fertilization failure, IVF and ICSI have very similar success rates. The added cost of ICSI (typically $1,000-2,000) is the main consideration. For patients with male factor, ICSI is meaningfully better.

What about embryo quality?

Studies have not shown meaningful differences in embryo quality between IVF and ICSI when sperm parameters are normal. For severe male factor, ICSI produces more usable embryos per cycle.

How to decide

Talk to your fertility specialist about your specific case. The decision is usually made jointly based on semen analysis results, any previous cycle outcomes, and the recommendation of the embryology team. Both techniques are well-established and produce healthy babies at the same rate.
